I/(we) hereby Authorize Rockingham Cooperative to store my (our) credit/debit card.  I/(we) further authorize Rockingham Cooperative to make repeated and/or unscheduled charges to my(our) credit/debit card for future purchases that I verbally or otherwise authorize from time to time and, if necessary, initiate adjustments for any transaction errors. This authorization will remain in effect until Rockingham Cooperative is notified by me to cancel this authorization. I further understand and agree to abide by Rockingham Cooperative refund policy related to this or any other purchases made with my credit/debit card on file. Any future changes made to this agreement will be sent via email at the address listed below.

Notice: You MUST call our corporate office Monday – Friday 8 AM to 5 PM at 540-434-3856 (e xtension 301) and verbally give your full card number over the phone. When calling, our staff will enter your card number into our accounting system and once entered, the card number will immediately become encrypted. At no time is card data written down or otherwise used in an un-encrypted format.
